Please note that in accordance with CAP’s Personal Health Information Privacy Policy, your personal health information is being collected and used for the sole purpose of planning, managing or administering programs or services. All personal information is stored in accordance with CAP’s privacy and retention policies or as otherwise required by law. From time to time you may receive mailings from us, from which you can opt out at any time. Any concerns with regards to the management of personal information can be directed to CAP’s Executive Director, who serves as the organization’s Privacy Officer.
