VON has been leading home and community care in Canada for over a century. VON was among the first organizations in the country to offer home care and public health nursing and now, over a century later, they provide home and community support services to over 10,000 people every day across Ontario and Nova Scotia.

We are a trusted registered charity who works hard to raise donations from individuals and corporations. We also receive pay-for-service fees from provincial governments; grants from federal, provincial and municipal governments and from foundations and other charitable organizations; and pay-for-service fees from community partners and recipients.  

We help our clients stay healthy, live well and remain active. We support those in frail and failing health and we contribute to the health and improved quality of life of Canadians in their homes and communities, where they want to be.   

Our Adult Day Programs (ADPs) provide social, recreational and therapeutic activities for seniors and individuals with a disability, in a safe and supervised environment.

Support and information about care giving is provided to individual family members or friends who are caring for a person. This service is offered either by telephone or an in-home visit. Click here to see programs.

With our eHomecare program, a registered nurse (RN) oversees and guides the work of a specially trained personal support worker, called a health-care technician (HCT). HCTs deliver care in the client’s home, acting as the eyes, ears and hands of the RN. They send data and observations to the nurse on a regular basis. The nurse assesses that information and provides instruction or direction to the health-care technician at the bedside. Click here to see programs.

The Meals on Wheels program allows clients to enjoy nutritious and affordable meals in the comfort of their own homes. Frozen meals are delivered Monday to Friday between 11:00 a.m. and 1:00 p.m. Please contact us for meal pricing. Click here to see programs.

Hospice volunteers provide emotional, social and spiritual support for those who are living with life-limiting or terminal illness and their families, so they may live at home, as comfortably as possible. Visits occur weekly for up to 3 hours. Click here to see programs.

Care is provided in the home for family/caregivers to take a break. Respite workers can provide companionship and supervision to temporarily relieve the demand for care and support. Activities include: Social engagement, personal care, monitoring, supervision, activation, light housekeeping (meal prep, laundry), medication assistance, and attendant care (support travel to medical appointments). Nursing clinics are a convenient way for clients to access wound care, medication administration, dressing changes, pain and symptom management, intravenous therapy and more, close to home. Click here to see programs.

Overnight respite allows family/caregivers to take a break while their loved one benefits from a program of special activity and support with personal care. Overnight stay is available from 4 p.m. to 4 p.m. (24hrs), seven days/week. Fee: $41.00 per overnight. Click here to see programs.

A specialized palliative nursing team whose staff supports clients facing life-threatening illness.  VON’s Palliative nurses are specially trained in Oncology (care of clients living with cancer) and Palliative Care (nursing with a focus on promoting optimum quality of life at all stages of a non-curable illness). Group exercise for senior adults (55+) who wish to improve their strength, balance and flexibility regardless of physical ability. The Gentle Exercise classes are available at various locations throughout Hamilton and surrounding area. Click here to see programs.

Telephone reassurance offers a regular telephone security check and social contact for seniors and adults with disabilities who may be living on their own. Click here to see programs.

Transportation services support seniors and adults with disabilities to attend medical appointments. VON requires a minimum of 3 business days’ notice and drives are based upon availability. Fee: $14.00 plus parking if applicable. Click here to see programs.

Volunteers provide a weekly social visit to seniors and adults with disabilities who would benefit from an increase in social activity. The compatibility of clients and volunteers is determined through a careful interview process. Click here to see programs.
